The Palliative Aged Care Coordinator works with providers of residential aged care facilities. The broad intent of this project is to increase knowledge and skills of providers of residential aged care facilities in a palliative approach to care using the ELDAC model, enabling them to:
- Care for and support residents with a life-limiting illness
- Develop and implement residents’ advance care plans and end-of-life care pathways, so supporting more residents to die in their place of choice.
ELDAC provides information, guidance, and resources to health professionals and aged care workers to support palliative care and advance care planning to improve the care of people in residential and community aged care settings. The project seeks to promote change enabling a reduction of avoidable hospital admissions, with shortened stays and improved quality of care for older Australians.
