National Advance Care Planning Week
16-22 March 2026

Advance care planning conversations should be routine and occur as part of a person’s ongoing healthcare plan. Better outcomes are experienced when advance care planning is introduced early as part of ongoing care rather than in reaction to a decline in condition or a crisis situation. Take the time during advance care planning week to start a conversation with your loved ones.

Advance care planning allows health professionals to understand and respect a person’s future healthcare preferences, for a time when they become seriously ill and unable to communicate for themselves.

All healthcare professionals and aged care workers have an important role to help with planning and ensuring people have choice in their care. You might want to start the conversation with someone, or they might ask you about the requirements of advance care planning. Ideally, advance care planning will result in a person’s preferences being documented into an advance care directive to help ensure these preferences are respected.

Click here to check out how to start a conversation.

See Advance care planning for more information and resources.

This annual day of action is dedicated to activating conversations and community actions around death, dying and bereavement.

It is important to explain that the national campaign known as Dying to Know Day, is more than just a ‘day’. It is a transformational campaign borne out of significant, strategic work focusing on ensuring that people have access to a good death, and that their wishes are at the forefront for others. The goal has always been to ensure that those who face the end of their lives, have their own wishes and experiences honoured by those who care, including the community within which they live.

Dying to Know Event – ‘Don’t let your end of life wishes go unspoken‘

This August due to high demand we will again be:

distributing ‘final arrangements starter packs’ – via GP’s, local libraries, retirement villages or by contacting us.

We used to talk about death. To prepare and plan for a future without us. It provided comfort and security for everyone involved, and drew us close together when we needed it most. The conversation was an act of love, bearing witness to a life well lived, and simply having it, could open doors to the soul.
