Referral to palliative care

Patients may self-refer or referral can be made by family, carers, the patient’s doctor (GP), other health professionals or clergy.  Patients, or their legal representative, must provide consent to being referred to palliative care.

Community members making a referral of a family member or friend should contact the palliative care service serving the area in which the patient lives.

Eligibility

Palliative care is available to anyone living with a life limiting illness, their families and carers, who require complex care and live in the service area of the palliative care service.

Fees and costs

There are no fees or costs for specialist palliative care.
